List of sheriffs office to take CCW application too?

Is there a list of places I can take my CCW application too? I live in Putnam county and can't seem to find this information anywhere.

You can apply for a ccw license in the county in which you live or an adjacent county. So any county that has a border with Putnam county. So that would be Paulding, Defiance, Henry, Hancock, Allen, or Van Wert.
